That doesn't seem to be it. I have a very clear repro case, but unfortunately it involves having the Xbox 360 SDK installed. When I click on the "Xbox neighbourhood" item in Windows Explorer and not all registered xboxes are turned on, the taskbar button for the corresponding explorer window disappears for a brief moment, and is then recreated at the right of all other existing buttons. I attached a DisplayFusion log of the event. I clicked on it at 15:21:15.
