It happens when different windows are activated/deactivated rapidly. I can easily reproduce it by repeatedly activating and deactivating two windows by clicking on the other window. The button then goes behind the titlebar and stays there for about a second or until the other window is activated. I had turned off the titlebar buttons, and now I can't get the bug to reproduce itself... It was showing the titlebar button over the tabs, though. I'll let you know if it happens again.

Regarding the TitleBar Button: It should only disappear if the window is maximized and there's a tab underneath the button. Is the button overlapping a tab?
