Just a quick follow-up to this. The latest DisplayFusion 5.1 Beta (<a href="http://www.displayfusion.com/Download/Beta/" target="_blank" rel="nofollow"><b>http://www.displayfusion.com/Download/Beta/</b></a>) has a new option on the Window Management tab for automatically moving child dialogs/modal windows to the same monitor as the parent application. I did notice in the log that you're loading a Monitor Profile using DisplayFusion, so I'm wondering if that has anything to do with it. If you leave DisplayFusion running, but set your monitor settings using the Windows Screen Resolution settings instead of loading the DisplayFusion Monitor Profile, does this issue still occur?
