I have the latest paid version 4.1 and I use the second monitor as a tv as well. When I switch to TV, the icons sometimes all move to the right side of the main monitor.

I have tried to save a icon position as "Single Monitor" but it does the same thing when I try to load it.

The only way to get them back to the left side is to turn on the second monitor and reload the icon settings.
It doesn't do this all the time but most of the time.

Is there a fix for this problem?

Windows some times moves the desktop icons around when monitor layouts change, or when docked applications (like DisplayFusion's taskbar), are added/removed.

In DisplayFusion 4.1, you can now assign Desktop Icon profiles to Monitor Profiles, so that they will be automatically loaded when the Monitor Profile is detected. Here are the basic steps:
  • Save both layouts as Monitor Profiles
  • Arrange the desktop icons for each layout and save them as Desktop Icon Profiles
  • Open the Monitor Configuration window and click "Manage Monitor Profiles"
  • Use the button at the bottom of the window to assign the Desktop Icon Profiles to their respective Monitor Profiles
