I'm not sure if that behavior is new but I only noticed it recently (on version 12.0.2 at the moment, Win 11 (25H2)).
When a window is on top, Alt + Tab will ignore that window as most recent window. Instead, I have to tab through the whole list to get focus back to that window.
So when window 1 (always on top) has focus, I Alt + Tab to window 2 and wnat to tab back, instead of window 1, window 3 is selected.
We have a similar issue on our list that's likely the same root cause. I've added your vote, we'll let you know if we have any news here going forward.
